Event Synopsis

Employee was driving the service road and cut the corner too much. This resulted in driver hitting the barricades on the TWY J/K closure, slightly bending one barricade and breaking the flashing light as well. Barricade was put back and in place light pieces picked up. Driver was coached/counseled on scene with his supervisor present.

Event Synopsis

The individual was driving the forklift, making their way from the inside of an open space, out to the confined space of the baggage drop off. As they approached the bay door, they lifted the forks slightly to miss the lip of the door. In doing so, the top of the forklift lifted up just enough so that when the individual pulled the forklift forward, it struck the fire suppression system. Once the individual realized what they hit, they stopped, backed up to prevent further damage, and called the AOC to report the accident

Event Synopsis

Systems lowered ladder to work on a belt. A cone was NOT placed in front of the ladder which is SOP. Driver went around the ladder and pulled to the right when he thought the 4th cart had cleared the ladder. I advised all CA personnel at the scene that the points can be appealed, but that the driver was going to hit the ladder regardless of a cone being placed by the ladder. UA personnel expressed frustration with ladders being in the way during a high operational tempo. Like all tenants, they are not authorized to move anything they are not trained on. We discussed maintaining situational awareness and the ability to stop an operation to have a hazard removed or otherwise addressed. At the end of the day, the driver cannot hit the ladder and must take every precaution to not damage anything. I discussed the crash with Arin Steed (CA), Gage Machado (CA), Connie Hyde (CA), the driver of the CA tug Francisco Rowe, and Travers Contreras (Systems). APD filed a traffic crash report.
